Foreign Workers' Medical Insurance

Documents needed for submission (general guidelines):

1. Duly completed claim form
2. Hospital Inpatient Discharge Summary
3. Worker's work permit card
4. All medical bills

Work Injury Compensation

Report to MOM within 10 days (if possible) after the accident occurs.

Documents needed for submission:

1. Duly completed claim form
2. Worker IC copy (Singaporean) or work permit card (non-Singaporean)
3. MCs given by the doctor
4. Medical bills
5. Past 12 months salary vouchers of the worker
